That pressure pushes against your eardrum from the outside in, and if the air pressure inside your middle ear doesn't match, you get that familiar, awful squeeze.

The issue? If you wait until you feel pain to equalize, you're already behind. The Eustachian tubes can actually close under pressure, which means forcing a Valsalva when you're already squeezed can over-pressurize your inner ear or even cause a reverse block on ascent.
Use Valsalva as a starting point, not a solution.
The Frenzel Technique — The Freediver's Best Friend
This is the one that changes everything for freedivers in particular, and many scuba divers swear by it too.
Instead of using lung pressure to push air up, the Frenzel technique uses the back of your tongue like a piston. You close your glottis (the back of your throat — the same thing you'd close if you were lifting something heavy), pinch your nose, and then move your tongue upward and back as if saying the letter "K" or "G." This pumps a small burst of air into the Eustachian tubes without any lung involvement.

Why does this matter for freedivers? Because on a breath-hold dive, you have a finite amount of air — and using lung pressure to equalize wastes it. The Frenzel technique lets you equalize efficiently and conserve your breath simultaneously.
Learning the Frenzel takes practice. Most people need a few sessions (or MANY SESSIONS) on land before it clicks. The Toynbee Maneuver
This one's less talked about but useful for equalization on ascent or in cases of a reverse block. You pinch your nose and swallow. Swallowing opens the Eustachian tubes briefly, and the jaw movement helps shift air pressure. It's mild, gentle, and great for equalization at shallower depths or when you're feeling sensitive.
The Golden Rules of Ear Equalization
Equalize Early and Often — Don't Wait for Pain
This is the most important rule, and the one most people ignore. Start equalizing before you even feel discomfort — at the surface, and then every meter or two on your descent. Waiting until pain hits means the tube may already be closed and locked.
Think of it like this: you're not fixing a problem, you're preventing one.
Descend Slowly and Stay in Control
Panic descents and uncontrolled drops are a fast track to ear problems. You need time to equalize — it's not instant. If you're in a hurry, your ears will pay for it. Freedivers use a controlled head-first descent; scuba divers should use a line if needed to control rate. Slow is smooth, smooth is deep.
Tilt Your Head

Some freedivers practice with a nose clip in the shower. Some use a mouthpiece mock-up. Others use a balloon (yes, look it up on youtube)...Whatever gets your Eustachian tubes opening reliably on land will translate underwater.
